Rodent and Small Mammal Transfer Request​
 
To facilitate the approval process and to maintain appropriate records, an animal transfer request must be submitted to the facility manager who will verify the species, strain, and number of animals against the approved IACUC protocols. The manager may approve animal transfers between protocols of an individual PI. Transfer of animals between two PI's requires the signature of the attending Veterinarian or his/her designee.  Download form​



Dog and Cat Transfer Request

Transfer of animals between two PI's requires the signature of the attending Veterinarian or his/her designee.​  All animals will transfer out of and return to the colony holding protocol.  The form can be emailed to OLAC or faxed to 4-5649.  Download form​​​​​​

 

Clinical Incident Report​
 
The Clinical Incident Report serves as initial notification of OLAC veterinary staff that an animal has received veterinary medical attention. The form should only be used in cases where OLAC veterinarians are not the veterinarian of record. Clinical Incident Report for Research and Education Centers​

The Clinical Incident Report serves as initial notification of OLAC veterinary staff that an animal or herd housed at one of the Institute's Research and Education Centers has received veterinary medical attention. For routine conditions, the form should only be completed when disease incidence is above that which is expected. Click here to electronically complete the form or fax downloaded form to 4-5649. Clinical Case Request

The Clinical Case Request serves as initial notification of OLAC staff that an animal requires veterinary medical attention. Click the link to access the online clinical case request form​ and sign in using your UTK netid and password.



Rodent Surgery/Procedure Record​

The Rodent Surgery/Procedure record serves as a template for recording all relevant animal information during the procedure and can be modified to include criteria of the laboratory's choosing. Download form​



Non-Agriculture Research Animal Acquisition

All acquisition, procurement, or movement of non-agricultural research animals to or from the University of Tennessee animal facilities or laboratories must be specifically approved in advance by the IACUC (via an approved protocol) and the Attending Veterinarian or his/her designee. To facilitate the approval process and to maintain appropriate records, an animal requisition form must be submitted to the facility manager who will verify the species, strain, and number of animals against the approved IACUC protocol. For requests to procure or otherwise acquire non-agricultural animals from non-regulated vendor sources (i.e. animals legally trapped, to be received from a collaborator, donated, acquired from unregulated sources) an animal requisition form must be submitted in advanced and approved by OLAC before the animals are procured.​  Download form​​



Receiving Rodents from Non-Approved Vendors​

Receipt of rodents from non-approved sources must be approved in advance by the Attending Veterinarian or his/her designee. The Rodent History Questionnaire​​​ and the Rodent Quarantine Procedures​​ forms must be completed to initiate importation of animals for non-approved vendor sources. The facility manager along with the principal investigator should complete appropriate sections of the Rodent History Questionnaire and the Rodent Quarantine Procedures forms. Completed forms should be emailed to clcarter@utk.edu or faxed to 865-974-5649.
